以安全科学理论为基础, 运用三类危险源理论,通过对煤矿生产系统状态及其危险源结构的分析研究,探讨了煤矿动态触发型危险源与静态固有型危险源的概念及分类;分析了煤矿人因事故是由动态触发型危险源引起煤矿静态固有型危险源爆发的结果。根据安全行为科学原理,将煤矿人因失误分为个体操作人因失误与组织管理失误;指出了动态触发型危险源是一种人因失误,静态固有型危险源是由于人因失误或组织管理失误所致。通过对湖南某矿历年发生的事故分析研究,发现该矿因组织管理失误而诱发的人因事故,所产生的后果比一般个体操作人因失误所产生的后果更为严重。应用安全系统工程理论对煤矿人因事故发生原因进行了详细分析与探讨,建立了煤矿生产系统人因事故形成逻辑关系图、煤矿人因事故发生原因的FTA逻辑模型图;指出了煤矿人因事故的形成是由于初级控制机制与次级控制机制共同失效的结果。运用灰色关联分析,对湖南某矿历年发生的事故做了人与事故的动态灰色关联分析,得出了该矿历年的事故引发或受害群体,绝大部分是工龄不足十年、文化程度为小学与初中的青年职工。根据该矿历年发生事故起数,运用灰色预测理论对该矿来年有可能发生的人因事故起数进行了科学的预测,并建立了切实可行的灰色预测模型。根据纵深防御原则,建立了煤矿人因事故纵深防御的“四阶段”预防管理模式;分析了加强组织规范、组织沟通、组织功能、组织文化氛围等是预防煤矿人因事故的有效途径。最后讨论了应用群体动力学技术对煤矿人因事故预防的策略。这有助于煤矿企业提高安全管理水平,预防人因失误,遏止或减少人因事故的发生。

This paper discusses the concept and classification of the dynamic touch-to-go risk source and static innate risk source in coal mine and analyzes accidents due to human factor based on theory of safety science and three kinds of hazard. The results show that the eruptive causation that the dynamic touch-to-go risk source bring about the static innate risk source in coal mine cause accidents due to human factor through analysis and research the state of production system and hazard structure of coal mine. According to safety behavior science principles, human error is divided into individual operation error and organizational management failures in coal mine. The paper pointed out that the dynamic touch-to-go risk source is a kind of human error and the static innate risk source results from human error or organizational management failures. It is found out that the aftereffect of accidents that result from organizational management failures are even more serious than the aftereffect of accidents that result from general individual operation error by analyzing the coal mine’ accidents in past years in Hunan. The article analyzes and discusses in detail the causes of accidents due to human factor in coal mine by applying theories of safety system engineering and establishes accident forming logic model chart of coal mine production system and FTA mode chart of coal mine accidents’ causes. It demonstrates that coal mine accidents due to human factor resulted from the invalidation of the first and the second controlling mechanism. The article makes the dynamic grey interrelation analysis of man and accident by applying the grey interrelation analysis to the mine’ accidents in past years in Hunan. This paper concluded that the majority of causing accidents or injured colony is the workers who serve fewer than ten year, schooling at primary school and junior middle school’ young workers during the past years. According to the number of accidents in past years at the coal mine, the number of accidents is forecasted in the future by applying the grey forecast theory. This paper has set up feasible grey system prediction models. According to the principle of prevention-in-depth , the paper set up coal mine the prevention-in-depth of accidents due to human factor " the four stages" prevention and management mode. We analyze several ways to prevent accidents due to human factor at coal mine through strengthening the criterion of organization, organization communication, the function of organization and the culture atmosphere of organization etc. Finally, This paper discusses and applies the colony dynamics technology tactics that prevents coal mine accidents due to human factor. This contribution will help colliery to raise safety management levels, to prevent human error and check or reduce emergence of accidents due to human factor at coal mine.
