随着移动终端设备的普及以及医疗领域对于信息实时性的要求,如何利用移动终端设备改进诊断手段,减少门诊等待和信息采集时间,改进医生与患者的沟通是社会信息和医疗发展的必然趋势。本文工作是以某三甲医院风湿免疫科室实际项目为背景进行开展。该科室在强直性脊柱炎诊断治疗方面居国内领先地位。由于此项疾病的特殊性,医务人员需要在门诊诊断以及后期长期的病情跟踪过程中提取并记录大量的患者病历数据和各种相关信息。针对诊疗过程中数据处理效率低下,诊断时间有限,不易进行病情跟踪等问题,科室确立了开发个人移动终端专项系统进行医疗辅助诊断的项目立项。
移动终端软件开发平台使用中国个人移动市场占有率比较高的iPhone和iPad IOS平台。在开发过程中,先根据科室实际需求对系统需求进行分析,收集整理大量的专业术语以及医患工作流程,并对IOS、restful Web service、基于java的Web应用开发等相关技术进行了学习研究。在此基数上,将系统开发分为客户端系统和Web系统开发。客户端系统使用MVC模型设计,由表示层、控制层、数据层组成了客户端三层架构,实现了初次随访、再次随访和历史记录三个功能模块。Web系统使用jersey框架结构,实现了用户管理、患者检索和个人信息管理三个功能模块,并为客户端提供web service服务。
目前基于IOS的强直性脊柱炎临床信息软件已开发完成,在系统功能性和非功能性测试后达到了设计标准。该软件在科室的实际临床使用中可靠稳健,表明了该系统的设计是可行且有效的。

With the development of the mobile terminal device and the popularity of the medical field requirements for real-time, how to use mobile terminal equipment to improve diagnosis means, reduce outpatient waiting and information acquisition time, improve the communication between doctors and patients is the inevitable trend of information society and the development of medical and health. The work of this paper is to carry out the background of the actual project of the Department of rheumatism immunity in a three grade hospital. The Department in ankylosing spondylitis diagnosis and treatment in the domestic leading position. Because of the particularity of the disease, the medical staff need to extract and record a large number of patients' medical records and various related information during the process of outpatient diagnosis and long-term follow-up. In the diagnosis and treatment process, the data processing efficiency is low, the diagnosis time is limited, not easy to carry on the condition tracking and other issues, the Department established the development of individual mobile terminal special system for medical aided diagnosis of the project project.
Mobile terminal software development platform for the use of Chinese personal mobile market share is relatively high iPhone and IOS iPad platform. In the development process, according to the actual needs of the Department to the system demand analysis, collected a large number of professional terminology and patient work flow, and the IOS, restful web service, based on Java Web application development technology for the study. On this base, the system development is divided into client system and Web system development. Client system using MVC model design, by the presentation layer, control layer, data layer composed of the client three tier architecture, to achieve the initial follow-up, follow-up and the history of three functional modules. Web system using the Jersey framework to achieve the user management, patient retrieval and personal information management three functional modules, and to provide the client with service web services.
At present, the clinical information software based on IOS has been developed, and the design standard has been reached after the functional and non functional testing of the system. The software is reliable and robust in clinical practice, and it shows that the design of the system is feasible and effective.
